The left series of pictures show how it goes into position;

slide the wire through the piercing hole, and gently rotate it into the correct location.

The right picture shows how it fits in an upper piercing hole. 

If intended to be worn in an upper hole, the wire would normally 

be attached to the front legs, making the dog face upward.
